Shocking footage shows the young boy clinging onto the roof racks of a silver Holden jeep as it sped through Perth, Australia at around 6pm on Friday.
Horrified onlookers desperately tried to get the mothers attention, while other drivers flashed their lights and honked horns, Nine News reported.
One told the Australian news service: "The child seemed fine and looked like he was enjoying himself."
But another said they "felt sick to the stomach" after seeing the baby on top of the car.
Police who are now investigating the incident, believe the mum got her child back in the car after being alerted by witnesses.
A spokesperson for Canning Vale Police said: "Police observed three children in the car, two secured in child restraint seats, however it will be alleged that a four-year-old boy was unrestrained."
It comes weeks after a young boy was hailed a hero for grabbing the steering wheel when his mum Lauren Smith, 27, blacked out on a busy dual carriageway.
Lauren started violently shaking as her Ford Ka smashed and scraped the central reservation.
But quick-thinking Ben sprang into action from the front passenger seat, guiding the vehicle across the two lanes to safety on a grass verge.
